Download it. You can choose to pay for it if you want to be a dedicated streamer for better audio quality and video quality, but it's free otherwise.

Simply connect your dazzle to your computer and select "Add..->Cameras->Dazzle DVC100" to get your dazzle to show on the stream. To record in game sound and mic sound at the same time, it's kind of difficult to do and a pain to set up, so most people just set up a mic and hopefully catch the in game sound from the TV in the mic along with voice.
